Đặt vấn đề: Gây tê tủy sống có kết hợp morphin tủy sống đã được nghiên cứu nhiều trong thực hành lâm sàng phẫu thuật vùng bụng. Trong phẫu thuật nội soi cắt đại, trực tràng cũng cần có những phương pháp giảm đau đa dạng, vì vậy, sử dụng morphin tủy sống để giảm đau sau mổ trong phẫu thuật này là một nhu cầu cần thiết trong thực hành lâm sàng. Kết quả nghiên cứu cho thấy mức độ giảm đau trong 24 giờ đầu sau mổ đạt hiệu quả tốt và có ít tác dụng không mong muốn. Mục tiêu: Đánh giá hiệu quả giảm đau sau mổ và các tác dụng không mong muốn của gây tê tủy sống với morphin kết hợp bupivacain sau phẫu thuật nội soi cắt đại, trực tràng. Đối tượng và phương pháp: nghiên cứu tiến cứu mô tả cắt ngang từ tháng 5 đến tháng 12 năm 2020 trên 63 bệnh nhân được gây tê tủy sống với 300µg morphin tủy sống phối hợp gây mê nội khí quản. Ghi nhận mức độ giảm đau theo thang điểm Visual Analog Scale (VAS) và các tác dụng không mong muốn sau mổ bao gồm buồn nôn và nôn, suy hô hấp, ngứa,… nếu có. Kết quả: Hiệu quả giảm đau khi nghỉ đạt 95,2%, khi vận động nhẹ đạt 88,9% với VAS ≤ 3. Các tác dụng không mong muốn 11,1%, bao gồm: nôn và buồn nôn 6,3%, ngứa 1,6%; có cả hai tác dụng ngứa và buồn nôn: 3,2%. Kết luận: Morphin tủy sống liều 300 µg có hiệu quả giảm đau tốt và an toàn cho phẫu thuật nội soi cắt đại, trực tràng.
