CLINICAL AND PARACLINICAL CHARACTERISTICS, ASSOCIATED FEATURES, AND TREATMENT OUTCOMES OF ECTOPIC PREGNANCY WITH ≥300 mL OF HEMOPERITONEUM RECORDED AT SURGERY IN CAN THO FROM 2024 TO MARCH 2026
Main Article Content
Abstract
Background: Ectopic pregnancy may cause hemoperitoneum and hemorrhagic shock. Describing surgically managed patients with a substantial volume of intraperitoneal blood may clarify their clinical manifestations and treatment outcomes. Objectives: To describe clinical and paraclinical characteristics and treatment outcomes, and to examine features associated with an intraoperative hemoperitoneum volume ≥300 mL among surgically managed ectopic pregnancies in Can Tho from 2024 to March 2026. Materials and methods: This retrospective cross-sectional study included 151 surgically managed ectopic pregnancies at two hospitals. The study outcome was operationally defined as ≥300 mL of intraperitoneal blood recorded at surgery. Results: Of 151 surgical cases, 64 (42.38%) had ≥300 mL of intraperitoneal blood. In this group, abdominal pain occurred in 95.31%, amenorrhea in 85.94%, vaginal bleeding in 67.19%, and moderate-to-large free intraperitoneal fluid on ultrasonography in 71.88%. An intraoperative hemoperitoneum volume of 500-1,000 mL was recorded in 57.81%; 46.88% received blood transfusion and 81.25% underwent laparoscopy. After adjustment, moderate-to-large intraperitoneal fluid on ultrasonography (aOR=10.39; 95% CI: 4.15-25.98; p<0.001) and posterior fornix tenderness (aOR=3.41; 95% CI: 1.28-9.11; p=0.014) remained associated with hemoperitoneum ≥300 mL. Conclusions: Among surgically managed ectopic pregnancies, 42.38% had ≥300 mL of intraperitoneal blood. Moderate-to-large free fluid and posterior fornix tenderness were concurrent manifestations of this outcome and should not be interpreted as causal risk factors, early-screening predictors, or evidence of delayed hospital presentation.
