Abstract

Background: Spondylosis is a common musculoskeletal condition that impairs physical function and quality of life and generates healthcare costs. Assessing direct medical costs and patients’ willingness to pay for health improvement may provide useful evidence for healthcare resource management. Objectives: To determine direct medical costs, willingness to pay for a hypothetical treatment aimed at improving quality of life, and related factors among hospitalized patients with spondylosis covered by health insurance at Hau Giang General Hospital during 2025– 2026. Material andMethods: A cross-sectional analytical study was conducted among 194 hospitalized patients with spondylosis covered by health insurance. Data were collected from medical records, itemized hospital bills, and face-to-face interviews. Multivariable linear regression was used to identify factors associated with willingness to pay. Results: The mean age was 70.95 ± 11.10 years, and women accounted for 66.5% of the study population. The mean direct medical cost per hospitalization was 2,759,135 ± 717,278 VND, of which health insurance covered 2,563,306 ± 693,780 VND, equivalent to 92.9% of the total cost. The mean annual willingness to pay for the hypothetical treatment was 13.36 ± 6.29 million VND. After multivariable adjustment, female sex and having no current spouse were associated with lower willingness to pay, whereas higher monthly income was associated with higher willingness to pay (p < 0.05). Conclusion: Health insurance covers the majority of inpatient treatment costs for patients with spinal degeneration. Willingness to pay is primarily influenced by gender, marital status, and income.
