THE PREDICTIVE VALUE OF THE APRI INDEX FOR SEVERE DENGUE IN CHILDREN AT CAN THO CHILDREN’S HOSPITAL IN 2025-2026

Abstract
Background: Dengue in children may rapidly progress to severe disease and lead to lifethreatening complications. Early identification of biomarkers associated with disease severity remains an important clinical concern. The Aspartate Aminotransferase to Platelet Ratio Index (APRI), which combines AST levels and platelet count, may reflect both hepatic injury and thrombocytopenia in Dengue infection. Objective: To evaluate the predictive value of APRI for severe Dengue in pediatric patients. Materials and methods: Diagnostic accuracy study was conducted on 129 children with Dengue admitted to Can Tho Children's Hospital from May 2025 to February 2026. The prognostic performance of APRI was assessed using receiver operating characteristic (ROC) curve analysis. Results: Among the 129 enrolled patients, 49 had severe Dengue and 80 had non-severe Dengue. The mean age was 11 ± 3.1 years. At the first assessment, the median APRI value was significantly higher in the severe group compared with the non-severe group [9.64 (1.07–200.19) vs. 1.13 (0.25–9.22)]. Similar findings were observed at the second assessment, with APRI values of 15.58 (1.81–190.15) and 2.14 (0.21–23.87), respectively. An APRI cutoff value of 3.19 at the first assessment yielded an AUC of 0.927, with 83.7% sensitivity and 85% specificity for predicting severe Dengue. At the second assessment, a cutoff value of 5.63 showed an AUC of 0.902, sensitivity of 87.8%, and specificity of 76.3%. Conclusion: APRI demonstrated good predictive performance for severe Dengue in children and may serve as a useful tool for early risk stratification in clinical practice.
