ASSOCIATION OF MATERNAL ANTHROPOMETRIC INDICES AND MEAN CORPUSCULAR VOLUME WITH NEONATAL JAUNDICE DUE TO INDIRECT HYPERBILIRUBINEMIA

Abstract

Background: Indirect hyperbilirubinemia in neonates is a common condition and may lead to serious complications if not treated promptly. Objectives: 1. To describe the clinical characteristics of neonatal indirect hyperbilirubinemia; 2. To investigate its associations with maternal anthropometric indices and maternal mean corpuscular volume (MCV). Materials and methods:  A cross-sectional descriptive study with analytic components was conducted in 220 neonates diagnosed with indirect hyperbilirubinemia at Can Tho City Obstetrics and Gynecology Hospital from 2024 to 2026. Results: Male infants accounted for 51.4%, term infants for 55.9%, normal-birth-weight infants for 67.3%, and cesarean deliveries for 69.1%. Jaundice was predominantly Kramer zone 4 (67.3%), with onset after 48 hours in 73.2% and duration of less than 7 days in 90.5%. Maternal weight <45 kg, height <155 cm, body mass index <18.5 kg/m², and MCV <80 fL were associated with jaundice onset within 48 hours. Maternal weight <45 kg and MCV <80 fL were associated with jaundice lasting ≥7 days. In the multivariable model, after adjustment for maternal BMI and gestational age, maternal MCV <80 fL remained independently associated with jaundice onset within 48 hours (aOR=2.80; p=0.015). Conclusions: Low maternal stature, low body weight, and low MCV were associated with jaundice onset within 48 hours and/or jaundice lasting ≥7 days in neonates with indirect hyperbilirubinemia. Maternal MCV <80 fL remained independently associated with jaundice onset within 48 hours after adjustment for maternal BMI and gestational age. In the context of excluding maternal iron deficiency, low MCV suggests microcytosis that warrants attention and may include causes related to hemoglobin disorders. Therefore, maternal MCV <80 fL may help identify infants who require closer monitoring and guide further evaluation for hematologic causes when clinically appropriate.
