EARLY OUTCOMS OF LAPAROSCOPIC SURGERY IN THE TREATMENT OF GASTRIC CANCER AT CHO RAY HOSPITAL

Abstract
Background: In Vietnam, laparoscopic total gastrectomy (LTG) for gastric cancer treatment has been announced; however, there have been no studies on LTG for total gastrectomy in gastric cancer treatment. Objective: To evaluate the outcomes of applying laparoscopic total gastrectomy in the treatment of gastric body cancer at Cho Ray Hospital. Materials and methods: A prospective descriptive study of 74 cases diagnosed with gastric cancer in the body region, who underwent total laparoscopic gastrectomy at the Department of Gastrointestinal Surgery - Cho Ray Hospital from October 2023 to October 2025. Results: We observed that males accounted for a higher proportion at 55.4%, with a male-to-female ratio of 1.2:1. The average age was 59 years. The rate of patients receiving neoadjuvant chemotherapy was 27.1%. Regarding imaging findings on CT scans, more than half of the patients had tumors located entirely in the gastric body region, with 20.3% having tumors extending to the greater curvature. Most tumors were smaller than 5 cm, accounting for 78.4%. The majority were adenocarcinoma with moderate differentiation on histopathological examination. The average surgery time was 266.9 minutes, with an average of 14 lymph nodes dissected during laparoscopic gastrectomy. Two intraoperative complications were recorded, accounting for 2.8%, with no cases requiring conversion to open surgery. Postoperative complications were recorded in two cases, with no mortality. The average time to first bowel movement after surgery was 4 days, and the average hospital stay was 7 days. Conclusion: Total laparoscopic gastrectomy for the treatment of gastric cancer is safe and effective, with a short recovery time and low rates of complications.
