Abstract

Background: The GeneXpert MTB/RIF assay (Xpert MTB/RIF) has been implemented and replaced traditional smear microscopy techniques for detecting Mycobacterium tuberculosis (MTB), marking a significant breakthrough in the fight against tuberculosis (TB) and rifampicinresistant tuberculosis (RR-TB). Objectives: 1. To determine the detection rate of new pulmonary tuberculosis (PTB) cases among suspected PTB case assay at Tien Giang Tuberculosis and Pulmonary Disease Hospital in 2022-2023; 2. To determine the value of GeneXpert MTB/RIF assay in detecting new PTB cases among suspected PTB cases. Materials and methods: This crosssectional descriptive study was conducted on 795 patients who had no history of pulmonary tuberculosis (PTB) or had never taken anti-TB drugs. These patients presented with abnormal clinical symptoms and chest x-ray findings suspected of PTB. The Xpert MTB/RIF assay and smear microscopy with Ziehl-Neelsen (ZN) staining were performed on sputum specimens to detect new pulmonary tuberculosis cases. Results: The detection rates of new pulmonary tuberculosis using the Xpert MTB/RIF assay and ZN smear microscopy, which were 29.6% (235/795) and 19.5% (155/795), respectively. The bacillary load categorized by the Xpert MTB/RIF assay was as follows: high (38.8%), medium (24.7%), low (28.1%), and very low (7.2%). The level of agreement between Xpert MTB/RIF and ZN smear microscopy was moderate (Kappa = 0.698). Conclusions: The Xpert MTB/RIF assay had a higher capacity for detecting new pulmonary tuberculosis in suspected pulmonary tuberculosis patients than ZN smear microscopy, and the level of agreement between both assays was moderate.
