EARLY OUTCOMES EVALUATION OF LAPAROSCOPIC PANCREATICODUODENECTOMY FOR PERIAMPULLARY CANCERS AT CAN THO CITY IN 2022 – 2024

Abstract

Background: Periampullary cancers represent 0.2% of all gastrointestinal tumors. Laparoscopic pancreaticoduodenectomy is a complex procedure with a high success rate, although there are still many complications. Objectives: To evaluate the clinical features of periampullary cancer and the early outcome of laparoscopic pancreaticoduodenectomy for periampullary cancer. Materials and methods: A prospective observation study was conducted on 31 patients who underwent laparoscopic pancreaticoduodenectomy for periampullary cancers from June 2022 to April 2024 at Can Tho City. Results: The mean operation time was 405 ± 83 minutes, with an approximate blood loss of 233 ± 126 ml. The success rate of laparoscopic pancreaticoduodenectomy was 84%. The postoperative hospital stay was 12 days. Major complications (Clavien – Dindo ≥ 3) were 22.7%. The favorable outcomes were 61.3%. In this study, 4 cases (13%) were graded as B, and C pancreatic fistula, with grade C leakage was 6.5%. Three cases experienced biliary fistula (9.7%), and all of them were stabilized with medical management. Two cases (6.5%) resulted in mortalỉy during the postoperative phase: one case had grade C pancreatic fistula and another one had severe pneumonia. Conclusions: Laparoscopic pancreaticoduodenectomy for periampullary tumors emerges as a feasible and efficacious surgical modality. Nevertheless, the incidence of complications remains substantial, emphasizing surgeons with extensive expertise.
