STUDY OF CLINICAL, PARACLINICAL AND EVALUATE THE INITIAL RESULTS OF PANCREATODUODENECTOMY FOR TREATMENT PERIAMPULLAR TUMORS IN CANTHO

Abstract
Background: Pancreatoduodenectomy is the optimal treatment, but it is still considered a complex surgery with many techniques to restore different anastomosis. Understanding the characteristics of periampullar tumors, studying and applying appropriate treatment techniques to minimize complications, surgical complications and mortality, improve the survival time after surgery. Objectives: Study clinical and paraclinical characteristics and evaluate the initial results of Pancreatoduodenectomy for treatment periampullar tumors. Materials and methods: A prospective description was performed in 34 patients who underwent pancreato-duodenectomy for treatment periampullar tumors from Juanuary 2018 to June 2019 at Cantho. Results: 34 patients with periampullar tumors had an average age of 59.59 ± 12.2 years old, the ratio of male / female ≈ 1.3. There are 29.4% cases of pancreatic head tumors, Vater tumors 52.9% cases, distal common bile duct tumors were 11.8%, duodenal tumors were 5.9%, adenocarcinoma was 33 cases accounting for 97.1%. The rate of internal bleeding after surgery was 5.9%, gastrointestinal bleeding and gastrointestinal fistula were 2.9%, pancreatic fistula accounted for 11.8%, wound infection accounted for 17.6%, there were 3 deaths after surgery accounted for 8.8%. In 34 cases, we recorded good treatment results, accounting for 64.7%, the average of 23.5% and the bad cases of 11.8%, with mortality rate accounted for 8.8%. Conclusion: Pancreatoduodenectomy is the only optimal treatment for periampullar tumors, the success rate is high in the early stages.
