EARLY OUTCOMES OF TOTALLY LAPAROSCOPIC PANCREATICODUODENECTOMY FOR PERIAMPULLARY CANCER: A RETROSPECTIVE STUDY OF 65 CASES

Abstract

Background: Laparoscopic pancreaticoduodenectomy for periampullary carcinoma remains a difficult procedure with high morbidity rate. Objectives: To estimate the feasibily, safety and effectiveness of this procedure performed in University Medical Center at Ho Chi Minh city, Vietnam. Material and methods: a retrospective study, case series report on 65 patients indicated for laparoscopic pancreaticoduodenectomy due to periampullary cancer of Vater, from February 2017 to January 2022 at University Medical Center at Ho Chi Minh city . Results: there were 65 patients underwent laparoscopic pancreaticoduodenectomy for periampullary carcinoma in University Medical Center. The mean operating time was 509 94 minutes. The estimated blood loss was 267 102 ml. The median hospital stay was 8 days. Major complications (Clavien-Dindo ≥ III)  and pancreatic fistula rates were 13.8% and 12.3%, respectively. The median comprehensive complication index (overall complication) was 8.7. Three patients required reoperation due to severe pancreatic fistula and bleeding. There was no death within 90 days postoperatively. The number of harvested lymph nodes was 15.8 ± 6.2. R0 resection achieved in 100% of cases. Conclusions: In appropriate patients, laparoscopic pancreaticoduodenectomy could be a feasible, safe and effective procedure for periampullary carcinoma treatment.
