VALUE OCCIPUT SPINE ANGLE IN ASSESSMENT OF LABOR AT CAN THO UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL IN 2023

Abstract
Background: A good birth prognosis helps physicians intervene at the right time and limit complications for trying to keep birth at the lowest level. Objectives: (1) To describe the clinical and paraclinical characteristics of labor and birth of full-term pregnant women hospitalized at the Department of Obstetrics and Gynecology, Can Tho University of Medicine and Pharmacy Hospital in 2022-2023; (2) To determine the value of fetal occiput-spinel angle ultrasound in assessment birth at the Department of Obstetrics and Gynecology, Can Tho University of Medicine and Pharmacy Hospital in 2022-2023. Materials and method: Case series study, there are 148 pregnant women with full-term singleton fetuses in labor at Can Tho University of Medicine and Pharmacy Hospital from August 1, 2022 to July 30, 2023 was examined to record position, Bishop's index, ultrasound to measure the occiput-spine angle, and monitor the pregnancy outcome. Results: Fetuses with left occiput-anterior position was 73%; Bishop ≥ 6 points accounted for 60% and Bishop < 6 points accounted for 40%; Average fetal weight estimated on ultrasound 3129.05±311.96 grams; The average occiput-spinel angle was 121.5±6.8 degrees. 73.7% of pregnant women gave birth naturally. Choosing the cutoff point of the occiput-spine angle was ≥ 120 degrees, the sensitivity in predicting vaginal birth was 79.4%; specificity was 63.4%; positive predictive value 85%; negative prediction 54.1%. Conclusion: The method of measuring the occiput-spine angle should be used to assess spontaneous vaginal delivery during labor.
