THE ESBL-PRODUCING STATUS OF KLEBSIELLA PNEUMONIAE AND VALUE OF DETECTION METHODS

Abstract
Background: The ESBL-producing status of Klebsiella pneumoniae is a severe problem at present and understanding about detection of ESBL production is very necessary. Objectives: To determine the rate of ESBL-producing Klebsiella pneumoniae by combined disk test and Vitek 2 compact system; To determine value of combined disk test and Vitek 2 compact system for detecting ESBL-producing.. Materials and methods: A cross-sectional descriptive study was conducted on 387 isolates of Klebsiella pneumoniae from clinical specimens of septic patients at Can Tho Central General Hospital from 01/2019 to 08/2019. ESBL was deteted by the combined disk test method and using the Vitek 2 compact system. The method for determination of minimum inhibitory concentration (MIC) was used as the reference assay. Results: The rate of ESBLproducing Klebsiella pneumoniae was 31.5% (122/387) detected by combined disk test and 31.3% (121/387) detected by Vitek 2 compact system. The sensitivity and specificity for detecting ESBLproducing were 93.44% and 97.35%. respectively with the combined disk test method and were 97.54% and 98.88% respectively with the Vitek 2 compact system; Two method have had a Kappa value 0,946. Conclusion: The rate of ESBL-producing Klebsiella pneumoniae at Can Tho Central General Hospital was in medium level. Both of methods for detecting ESBL-producing (combined disk test and Vitek 2 compact system) were the same value and highly compatible with each other (kappa=0,946).
