EFFECTIVE TREATMENT OF ADVANCED - STAGE NON SMALL LUNG CELL CANCER WITH EGFR MUTATIONS: A CLINICAL CASE AND OVERVIEW OF TREATMENT

Abstract

Background: In recent years, therapy for lung cancer treatment have been developing rapidly, with many new molecular targets in the treatment of lung cancer have been discovered such as EGFR, ROS1, KRAS, ALK, PD-L1...which help to prolong patients’ survival time and and improve patient's quality of their life. Of those Tyrosine Kinase Inhibitors (TKIs) of the Epithelial Growth Factor Receptor (EGFR) have been extensively studied and used for cancer patients nationwide and worldwide. Patients with Non-Small Cell Lung Cancer (NSCLC) harboring this EGFR mutation usually show an effective response to the targeted therapy. In clinical trials, the use of targeted drugs resulted in statistically significantly higher progression-free survival (PFS) compared with chemotherapy. In addition, the level of safety of the drugs has also been proved to be higher, overcoming the limitations of using chemical drugs such as the lack of specific selectivity for each individual and many side effects affecting the treatment cylce as well as it is effectiveness with chemotherapy. Case report: A 65-year-old female patient diagnosed with advanced - stage lung adenocarcinoma, having brain and pleural metastases, who habours the L858R point mutation of the EGFR gene. The patient was treated with Gefitinib and underwent Gamma Knife Radiosurgery, who is currently in partial response after 14 months of treatment and continues to be monitored for response to treatment. Conclusions: Treatment options for advanced stage non-small cell lung cancer depend on: Histopathologic type, EGFR mutation, patient's physical condition and economy.... Gefitinib is one of the effective target drugs in the first-line treatment for lung adenocarcinoma patients with EGFR gene mutations. It does improve the quality of patients’ life and prolongs survival time with low and acceptable toxicity.
