Tăng thanh thải là hiện tượng phổ biến trên những bệnh nhân viêm phổi nặng điều trị tại bệnh viện. Việc sử dụng các chế độ liều chuẩn của các kháng sinh trên những bệnh nhân này có thể không đảm bảo được nồng độ thuốc trong máu, dẫn đến không tối ưu được mục tiêu điều trị, gia tăng thời gian nằm viện và thúc đẩy đề kháng thuốc. Do đó, đánh giá nguy cơ và điều chỉnh chế độ liều là cần thiết nếu bệnh nhân có tăng thanh thải. Các kháng sinh thường dùng trong điều trị viêm phổi nặng có phân tầng nguy cơ cao như carbapenem, fluoroquinolon, aminoglycosid, vancomcin, linezolid. Dữ liệu về chế độ liều kháng sinh này cho bệnh nhân viêm phổi có tăng thanh thải tại Việt Nam còn hạn chế, chủ yếu dựa trên các tính toán từ mô hình dược động học quần thể. Do vậy việc áp dụng cần cân nhắc thận trọng hoặc thực hiện dưới giám sát nồng độ thuốc trong máu. Các nghiên cứu chuyên sâu hơn cần được thực hiện trong tương lai để tối ưu hóa điều trị để nhóm đối tượng bệnh nhân này.
