THE RESULTS OF SURGICAL TREATMENT OF DE QUERVAIN TENOSYNOVITIS

Abstract

Background: De Quervain tenosynovitis is common. This disease is caused by the inflammation of the synovium of tendon of the abductor pollicis longus and extensor pollicis brevis. The most important sign is pain at the radial  styloid process, exacerbated by thumb moverment and radial and ulnar deviaton of the wrist, definitive diagnosis base on Finkenstein test positive. There are many methods to treat this disease, such as conservative with thumb spica braces, oral non-steroid anti-inflammatory drugs (NSAIDs), or steroid injection into the first dorsal compartment. The surgical treatment is chosen when nonoperative treatment methods are failed. Objective: To evaluate the results of surgical treatment of De Quervain tenosynovitis disease. Materials and method: A descriptive cross-sectional study was conducted on 40 patients at Tam Minh Duc General Hospital from 01/2020 to 06/2021. Results: All 40 patients (2 men anh 38 women) with a mean age of 50.05 ± 13.57 years (20 -76 years old) had complete relief of pain and swelling over the first dorsal compartment, the VAS scores improved significantly at 03 month postoperative. The result of Finkelstein’s test was negative in all cases. The thumb’s range of movement was expected, the angles of extension and abduction of the thumb were standard. There were no cases of tendon disclocation, infection, neuroma. A subcompartment septum was found in 25 (62.5%) cases. The average operative time was 15.48 ± 6.51 min. Conclusions: Surgical treatment  is suitable for patients with  abductor pollicis longus and extensor pollicis brevis tenosynovitis after nonoperative management methods are failed.
