DIRECT MEDICAL COSTS AND HEALTH-RELATED QUALITY OF LIFE OF TWO ORAL ANTIDIABETIC REGIMENS FOR TYPE 2 DIABETES OUTPATIENTS AT HOC MON REGIONAL GENERAL HOSPITAL IN 2024-2025

Abstract
Background: Type 2 diabetes increases economic burden and adversely affects healthrelated the quality of life. Objectives: To determine direct medical costs and assess quality of life of metformin + gliclazide and metformin + sitagliptin regimens. Materials and methods: A crosssectional study combining retrospective and prospective data was conducted on 344 outpatients (172 per group) at Hoc Mon Regional General Hospital during 2024-2025. Direct medical costs were calculated from the payer’s perspective over 12 months. Quality of life was assessed using EQ-5D5L and converted to utility using the Vietnamese value set. Results: The metformin + sitagliptin group had significantly higher annual direct medical costs than the metformin + gliclazide group (5,708,412 [5,164,273 – 6,412,484] vs. 5,038,288 [4,366,162 – 5,911,285] VND; p<0.001), mainly due to drug costs. Median utility was significantly higher in the metformin + sitagliptin group (0.737 [0.669 – 0.847] vs. 0.723 [0.669 – 0.801]). No significant difference was observed in EQ-VAS scores. Conclusions: The metformin + sitagliptin regimen increases cost but improves utility, providing evidence for cost–utility analysis and formulary management.
