EVALUATION OF PHACOEMULSIFICATION SURGERY RESULTS WITH MONOFOCAL TORIC INTRAOCULAR LENS IMPLANTATION FOR ASTIGMATISM CORRECTION IN CATARACT PATIENTS

Abstract

Background: The proportion of cataract patients with significant corneal astigmatism is very common, requiring surgery to not only replace the opaque lens but also correct the astigmatism to achieve optimal vision. Objectives:  To evaluate the results of phacoemulsification surgery with monofocal toric intraocular lens implantation for astigmatism correction in cataract patients at Saigon Can Tho Eye Hospital from July 2024 to December 2025. Materials and methods: A prospective descriptive study without a control group was conducted on 40 eyes of 31 patients. Results: The mean patient age was 62 ± 9.62 years. Uncorrected distance visual acuity (UDVA) improved significantly from 1.00 ± 0.28 logMAR preoperatively to 0.13 ± 0.05 (1 week), 0.11 ± 0.05 (1 month), and 0.09 ± 0.05 (3 months) postoperatively. Corrected distance visual acuity (CDVA) also increased from 0.28 ± 0.23 logMAR preoperatively to 0.09 ± 0.04 logMAR at 3 months. Residual astigmatism decreased sharply from 1.62 ± 0.76 D preoperatively to 0.03 ± 0.14 D (1 week). The IOL demonstrated stable rotational alignment, with mean rotations of 1.32° (1 week), 0.38° (1 month), and 0.22° (3 months); 92.5% of eyes had rotation ≤ 5° and no cases had rotation ≥ 10°. Conclusions: Phacoemulsification with monofocal toric intraocular lens implantation provides significant visual acuity improvement, markedly reduces residual astigmatism, and demonstrates high rotational stability, proving it to be an effective, safe, and feasible method for treating cataracts with astigmatism.
