SURGICAL TREATMENT FOR LUMBAR INSTABILITY BY TRANSFORAMINAL LUMBAR INTERBODY FUSION

Abstract

Background: Lumbar instability is an important cause of low back pain, restricting mobilization and activities. There are many surgical methods indicated when conservative treatment fails, in which, transforaminal lumbar interbody fusion shows many advantages such as minimizing neurological complications and limiting blood loss. Objectives: To describe clinical characteristics, radiological features and to evaluate the results of surgical treatment for lumbar instability by transforaminal lumbar interbody fusion. Materials and methods: A cross-sectional study of 44 patients with lumbar instability underwent transforaminal lumbar interbody fusion at Can Tho University of Medicine and Pharmacy Hospital from January 2020 to December 2022. Results: The male to female ratio was 1:1.59, the average age was 58.48 ± 9.53. The mean duration of symptoms was 20.66 ± 14.53 months. L4-L5 level instability was the most common, accounting for 49.37%. Preoperative JOA score: 12.7 ± 1.29. Average surgery time: 178.41 minutes, average blood loss: 372.50 ml. There were 20 cases requiring blood transfusion during and after surgery. Recovery rate according to the JOA scale at 6 months after surgery was at good: 90.9%, fair: 9.1%. All 44 cases recorded grade II bone healing according to Bridwell, no cases of bone destruction were recorded. Conclusion: Transforaminal lumbar interbody fusion has very positive results and is currently the optimal treatment for lumbar instability.
