MEDICATION ADHERENCE AND ITS ASSOCIATION WITH TREATMENT REGIMENS AMONG OUTPATIENTS WITH TYPE 2 DIABETES AT A PROVINCIAL HOSPITAL IN LAM DONG PROVINCE

Abstract
Background: Medication adherence is a key factor in controlling type 2 diabetes mellitus (T2DM). However, data from the Central Highlands region of Vietnam, which has a high proportion of ethnic minorities and limited healthcare infrastructure, remains scarce. Objective: To determine the rate of medication adherence and its association with treatment regimens among outpatients with type 2 diabetes at Dak Nong General Hospital. Materials and methods: A cross-sectional descriptive study was conducted on 403 patients from March 2023 to June 2023. Medication adherence was assessed using the Morisky Medication Adherence Scale (MMAS-8). Data were analyzed using SPSS version 26. Results: The overall medication adherence rate was 69.2% (n=279). The main non-adherent behaviors included difficulty remembering to take multiple doses (61.0%), forgetting to take medication (54.1%), and stopping medication when blood glucose was under control (30.5%). Logistic regression analysis showed that patients using insulin had significantly lower adherence compared to the monotherapy group (OR = 0.401; 95% CI: 0,164– 0.983; p = 0.046), while patients using two or more drugs tended to have lower adherence, but this association only reached marginal significance (OR = 0.598; 95% CI: 0.358-1,.000; p = 0.050). Conclusion: The medication adherence rate among patients with type 2 diabetes at Dak Nong General Hospital was modest (69.2%), with significant differences across treatment regimens. Patients on monotherapy or insulin-containing regimens had particularly low adherence. Targeted interventions are needed to improve adherence, especially for these high-risk groups.
