EVALUATION OF TREATMENT OUTCOMES OF STAGE IIIB–IV NON- SMALL CELL LUNG CANCER WITH EGFR MUTATIONS USING FIRST-GENERATION TYROSINE KINASE INHIBITORS AT CAN THO CITY IN 2020 - 2023

Abstract

Background: Advanced non-small cell lung cancer (NSCLC) is often diagnosed late, with varied symptoms and distant metastases, leading to poor outcomes. EGFR mutations are common in Asian patients (10–50%) and respond well to tyrosine kinase inhibitors (TKIs). First-generation TKIs can improve survival and quality of life with good tolerability. Objectives: To evaluate clinical characteristics, treatment outcomes, and related factors in stage IIIB–IV NSCLC patients treated with first-generation TKIs in Can Tho (2020–2023). Materials and methods: A total of 58 patients with stage IIIB–IV EGFR-mutant NSCLC received first-generation TKIs at Can Tho Oncology Hospital and Can Tho University of Medicine and Pharmacy Hospital. Results: The mean age was 63.6 years, with a female predominance. Common symptoms included chest pain and dyspnea. Over half had metastases to ≥2 organs, mainly bone and pleura. Exon 19 deletion (60.4%) and L858R mutation (36.2%) were predominant. The overall response rate was 71.5%, and the disease control rate was 95.3%. Median progression-free survival (PFS) and overall survival (OS) were 16.1 and 21.2 months. Mutation type and skin rash were associated with survival outcomes. Adverse events were mostly mild, including rash and diarrhea. Conclusion: First-generation TKIs are effective, improve survival and quality of life, and are well tolerated in advanced EGFR-mutant NSCLC.
