EVALUATE THE DIFFERENCE IN CLINICAL AND PARACLINICAL CHARACTERISTICS IN PATIENTS WITH ACUTE MYOCARDIAL INFARCTION OVER AND UNDER 65 YEARS OLD AT HOAN MY CUU LONG GENERAL HOSPITAL IN 2021 - 2022

Abstract

Background: The presentation of acute myocardial infarction (MI) in elderly patients ( 65 years) is generally atypical, and the acute diagnosis is often missed. Objective: Determine the difference in clinical and subclinical characteristics in patients with acute myocardial infarction over and under 65 years old. Materials and methods: A cross-sectional descriptive study on a total of 86 patients being treated for acute myocardial infarction at Hoan My Cuu Long General Hospital. Results: Mean age was 66.55±11.01. Male patients predominate in both age groups. The median length of stay in the hospital of the group  65 years old was 9.46±5.16, higher than the group < 65 years old was 8.25±5.34. Characteristics of chest pain: patients < 65 years old had more typical chest pain than patients ≥ 65 years old (p=0.027). Sweating was more common in the group < 65 years old (p=0.015). Coronary artery disease risk factors between the two groups: prevalence of hypertension in the group ≥ 65 years old were higher than the group < 65 years old (92% vs 72.2%, with p=0.014), the proportion of patients with obesity accounted for quite high in the group < 65 years old compared with the group ≥ 65 years old (22.2% compared with 6%, with p=0.026. In terms of clinical characteristics: the electrocardiogram recorded in the group of patients  65 years old predominately no ST elevation at 68% (p=0.015). The proportion of patients with reduced left ventricular ejection fraction in the group of patients  65 years old was always higher than the group of patients < 65 years old (p=0.046). Conclusions: In a cross-sectional descriptive study on a total of 86 patients, the results noted that there were differences in clinical and laboratory characteristics of acute myocardial infarction in 2 groups of patients over and under 65 years of age.
