Abstract

Background: Chronic apical periodontitis often presents with subtle symptoms, making it easy to overlook. Radiographic diagnosis of periapical lesions plays a key role in determining the condition of the disease. Modern diagnostic technologies such as X ray images, and CT scans, especially Cone beam computed tomography allow for three-dimensional evaluation of lesions, providing dental specialists with a comprehensive view of the extent and range of damage. Objectives: To investigate the clinical and paraclinical characteristics of permanent incisors with chronic apical periodontitis treated at Can Tho Eye and Odonto - Stomatology Hospital from 2023 to 2025. Materials and methods: A cross-sectional descriptive study was conducted on 46 patients with 60 permanent incisors showing chronic apical periodontitis with lesion diameters <10mm. Results: The study was conducted on 60 permanent incisors with chronic apical periodontitis, with the maxillary central incisors accounting for the highest proportion (53.3%). The most common cause was dental caries (50%), followed by trauma (21.7%). Common symptoms included tenderness to percussion (51.7%) and formation of a sinus tract (36.7%). The average maximum lesion size was 5.8 ± 2.1mm. Conclusion: The study indicates that maxillary central incisors are the most commonly affected teeth with chronic apical periodontitis, mainly due to dental caries. Cortical bone destruction was common. Cone beam computed tomography played a crucial role in assessing lesion extent and enhancing diagnostic accuracy.
