EVALUATION OF SURGICAL OUTCOMES OF IMPACTED MANDIBULAR THIRD MOLAR EXTRACTION BASED ON DISTAL HEALING OF THE SECOND MOLAR AFTER HORIZONTAL MATTRESS VERSUS SIMPLE SUTURE CLOSURE AT NGUYEN TRAI HOSPITAL, HO CHI MINH CITY IN 2024-2025

Abstract
Background: Impacted mandibular third molars harm periodontal health of second molars' distal aspect. Primary wound healing is crucial for prognosis, with horizontal mattress sutures being more effective than simple sutures. Objectives: To evaluate the primary wound healing and distal periodontal status of second molars after impacted mandibular third molar surgery between mattress and simple suture groups at 1 and 3 months. Material and methods: A study on 36 patients with bilaterally impacted mandibular third molars. Each side was sutured with either horizontal mattress or simple sutures. Healing (CSR scores at 7/14 days) and periodontal health (PlI, GI, PD, alveolar crest height) were evaluated pre-surgery, 1, and 3 months post-surgery. Results: In the mattress suture group, CSR scores were 2.67 ± 0.16 on day 7 and 4.50 ± 0.25 on day 14; In the simple group, they were 1.33 ± 0.25 and 1.83 ± 0.30. At 3 months postoperatively, Plaque Index (PlI) decreased from 2.39 ± 0.11 to 1.83 ± 0.06 in the simple group and from 2.39 ± 0.12 to 1.44 ± 0.08 in the mattress group; Gingival Index (GI) reduced from 1.83 ± 0.15 to 1.39 ± 0.11 in the simple group and from 1.83 ± 0.13 to 1.17 ± 0.12 in the mattress group; Probing Depth (PD) decreased from 5.56 ± 0.21mm to 5.17 ± 0.17mm in the simple group and from 5.50 ± 0.14mm to 4.56 ± 0.10mm in the mattress group; The distance from the cementoenamel junction to the alveolar crest declined from 4.42 ± 0.17mm to 4.31 ± 0.17mm in the simple group and from 4.29 ± 0.16mm to 4.02 ± 0.15mm in the mattress group. Conclusion: Mattress sutures are more effective than simple sutures in promoting healing and improving periodontal outcomes distal to the second mandibular molar after third molar surgery.
