SURVEY OF THE MORPHOLOGICAL FEATURES OF THE SPHENOID SINUS AND THE ANATOMICAL STRUCTURES OF THE INTERNAL CAROTID ARTERY AND OPTIC NERVE IN RELATION TO THE SPHENOID SINUS ON CT SCAN OF THE PARANASAL SINUSES AT CAN THO IN 2023-2025

Abstract
Background: The investigation of the morphological features of the sphenoid sinus and its relationship with the internal carotid artery and optic nerve on computed tomography scans of the paranasal sinuses not only enhances the understanding of the anatomy of this region but also plays a crucial role in sphenoid sinus endoscopic surgery. Objectives: To evaluate the sphenoid sinus pneumatization, anatomic variations of internal carotid artery, optic nerve structures. Materials and methods: Cross-sectional study of 60 sinus CT scan at Can Tho General Hospital and Can Tho University of medicine and pharmacy hospital from June 2023 to february 2025. The image is reconstructed and measured by Radiant dicom viewer. Results: The sphenoid sinus pneumatization types were cyst-like (6.7%), pre-sellar (15%), and sellar and post-sellar (78.3%). The morphology of the internal carotid artery was protruding in (59.1%), non-protruding in (33.3%), and showed bony dehiscence in (7.6%). The morphology of the optic nerve was protruding in (54.1%), non-protruding in (41.6%), and showed bony dehiscence in (4.3%). The incidence of the sphenoid sinus septum attaching to the internal carotid artery was (20%). The sphenoid sinus ostium was located 100% in the sphenoethmoidal recess. The average distance from the sphenoid sinus ostium to the anterior nasal spine was 54.6 ± 4.8mm. Conclusion: The most common anatomical variation of the sphenoid sinus is the sellar and post-sellar type, accounting for 78.3%. The anatomical configurations of the internal carotid artery and optic nerve with bony defects are the rarest variations, occurring in 7.6% and 4.3% of cases, respectively, and are considered the most dangerous for patients. Identifying the location of the sphenoid sinus ostium and the average distance from the ostium to the anterior nasal spine helps surgeons avoid serious complications during surgery.
