HOME MEDICAL CARE NEEDS AFTER HOSPITAL DISCHARGE FOR ELDERLY PATIENTS AT CAN THO UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL

Abstract
Background: Vietnam's population is rapidly aging, causing increased demand for medical care for the elderly. However, home medical services have not yet fully met this need. Along with the overload in hospitals and the need to minimize inpatient treatment costs, medical care at home after discharge from the hospital is becoming an important trend. Objective: To determine the need for medical care at home after providing inpatient treatment to the elderly at Can Tho University of Medicine and Pharmacy Hospital from January 2024 to June 2024. Materials and methods: A cross-sectional descriptive research was conducted on 104 patients aged 60 years. Results: Elderly with low, medium, and high physical care needs at home were 76.9%, 17.3%, and 5.8%, respectively; Low, and medium psycho-social care needs were 98.1% and 1.9%. 14.4% of patients in the study needed care when necessary.
