EVALUATION OF THE TREATMENT OUTCOMES OF CLOSED DISTAL CLAVICLE FRACTURES USING THE HOOK PLATE FIXATION METHOD

Abstract

Background: Distal clavicle fractures are a clinical injury of the shoulder region with various treatment methods, they account for about 15-30% of all clavicle fractures, but the nonunion rate is high, approximately 30-45% with conservative treatment, leaving many persistent sequelae for patients. To address this issue, surgical fixation of distal clavicle fractures using a hook plate has been chosen by many surgeons. Objectives: To survey the clinical, paraclinical characteristics, and treatment outcomes of patients with closed distal clavicle fractures treated with hook plate method. Materials and methods: We conducted a cross-sectional descriptive study on 34 patients aged 16 and above with outer clavicle fractures treated with hook plate fixation surgery. Postoperative functional recovery was evaluated using the Constant score, with follow-up ranging from first to sixth months. Results: The average age of the patients participating in the study was 42.26 years. The primary cause of fractures was traffic accidents, accounting for 82.35%. All patients' surgical wounds healed well after 1 month of follow-up. After 3 months, 88.24% of patients showed bone union, reaching 97.06% bone union after 6 months. Regarding shoulder joint function, the Constant-Murley score reached 84.41 points after 3 months and 97.94 points after 6 months. Conclusion: Treatment of closed distal clavicle fracture using hook plate fixation surgery yields good results in terms of anatomical restoration and bone healing, with patients experiencing early recovery of shoulder joint function. This indicates that the hook plate is an appropriate for treating distal clavicle fractures.
