CLINICAL, PARACLINICAL AND THE EARLY RESULTS OF NON-OPERATIVE MANAGEMENT OF BLUNT HEPATIC TRAUMA AT CAN THO CENTRAL GENERAL HOSPITAL IN 2022-2024

Abstract
Background: In blunt abdominal trauma, liver injury is a frequent emergency, ranking second after spleen injury. With computed tomography, the classification and assessment of the level of damage in liver injury is more accurate, creating bases for the decision of possible non-operative management that could bring good treatment results for the patient. Objectives: To evaluate the results of nonoperative management of blunt hepatic trauma. Materials and methods: A descriptive cross-sectional studies was conducted on 54 patients treated for liver injury at Can Tho Central General Hospital from 8/2022 to 3/2024. Results: 54 patients treated for liver injury (36 men accounting for 66.7% and 18 women accounting for 33.3%). Mean age was 35.39±13.31. The main cause was traffic accidents, accounting for 94.4%. The majority of patients admitted to the hospital were hemodynamically stable, accounting for 96.3%. Regarding liver injury grading on CT according to AAST 2018, there were 30 grade III patients accounting for 55.6%, 23 grade II patients accounting for 42.6%, 1 grade I patient accounting for 1.9%. There were combined injuries in the abdomen accounting for 24.1%, mainly kidneys accounting for 91.7%. The success rate of non-operative management for grade I and II liver injuries is 100%, grade III accounting for 96.7%. The mean hospital stay was 9.19±4.55 days. Conclusions: Treatment of grade I, II, III liver injury according to AAST 2018 due to trauma with nonoperative management at Can Tho Central General Hospital achieved good results.
