STUDY ON CLINICAL AND PARACLINICAL CHARACTERISTICS OF MANDIBULAR MOLAR TEETH WITH PERIAPICAL LESIONS INDICATED FOR EXTRACTION AND EVALUATION OF IMMEDIATE IMPLANT PLACEMENT OUTCOMES

Abstract
Background: Implants provide good masticatory function, do not damage adjacent natural teeth, prevent the process of bone resorption, and have a long lifespan within the jawbone. Objectives: To investigate the clinical and paraclinical features of mandibular molar teeth with periapical lesions indicated for extraction and to evaluate the outcomes of immediate implant placement. Materials and methods: A cross-sectional clinical intervention study without a control group on 14 mandibular molar teeth with periapical lesions indicated for extraction and treated with immediate implant placement at Can Tho University of Medicine and Pharmacy Hospital in 2023 - 2024. Results: There were 2 positions for tooth extraction and implant placement: tooth 36 (50%) and tooth 46 (50%). The dimensions of the lesions had an average length, width, and height of 4.01 ± 2.33 mm, 3.31 ± 1.76 mm, and 4.03 ± 2.34 mm, respectively. Treatment results showed an average implant insertion torque of 34.29 ± 3.31 N/m, with all patients experiencing only mild pain and bone grafting in 93% of cases. Six months after implant placement, the level of bone resorption around the neck of the implant on both the mesial and distal sides was higher than at the 3-month mark with p=0.021. The implant stability index after 6 months also improved compared to the initial time (71.71 ± 1.64 versus 64.36 ± 3.65, p=0.0395), with most patients having a healing time of more than 6 months (71.4%). Conclusion: All patients experienced only mild pain after treatment. Six months after implant placement, the level of bone resorption around the neck of the implant was higher than at the 3-month mark. The implant stability index after 6 months also improved compared to the initial time.
