RESULT OF TREATMENT CHRONIC ANAL FISSURE BY CLOSED LATERAL INTERNAL SPHINCTEROTOMY AT CAN THO UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL IN 2020-2022

Abstract

Background: Chronic anal fissure is disease that is the third common in anorectal diseases. It makes the patients feel very painful and uncomfortable and it decreases their quality of life. There are a lot of therapy to treat. Closed lateral internal sphincterotomy has been proven as an effective operation with many advantages. Objectives: The research aims to describe clinical features and results of treatment chronic anal fissure by closed lateral internal sphincterotomy at Can Tho University of Medicine and Pharmacy Hospital. Materials and methods: A cross-sectional prospective study was conducted with 34 chronic anal fissure patients who underwent closed lateral internal sphincterotomy at Can Tho University of Medicine and Pharmacy Hospital from 2020 to 2022. Results: There were 34 patients. They were treated for chronic anal fissure disease by closed lateral internal sphincterotomy. The mean age was 32.35±10.9 years and male (47.1%), female (52.9%). The anal fissure was almost found in the posterior-6 hours (61.8%), in the anterior -12 hours (14.7%) and both (23.5%); The edge of fissure was high in 34 cases (100%). Sentinel skin tag was found in almost all cases (94.1%), while hypertrophied anal papilla was found in 7 cases (20.6%). Hemorrhoids and anal polyps associated with anal fissures accounted for 26.5% and 5.8% respectively. Visual Analog Scale (VAS) preoperatively was 5.85±0.8 and at one week postoperatively was 2±0.7 (p< 0.0001). There were no serious complications after surgery, but there was a case that had a complication of urinary retention. All patients discharged in the first postoperative day. Healing of fissure after 4 weeks was 88.2% and after 8 weeks were 100%. No case of recurrence was observed after 6 months. Conclusion: Close lateral internal sphincterotomy is not only effective for the treatment of chronic anal fissure but also safe. It reduces the postoperative pain and hospital stay.
