IMMATURE PLATELET FRACTION INDICATOR IN EARLY PREDICTION OF PLATELET RECOVERY IN DENGUE DISEASE IN ADULTS WITH THROMBOCYTOPENIA

Abstract

Background: Thrombocytopenia is a common complication in dengue that sometimes necessitates platelet transfusion. Immature platelet fraction (IPF), which can be measured using a haematology analyser, is a precursor indicating platelet formation in the bone marrow, and is helpful in predicting platelet recovery in dengue patients with thrombocytopenia. Objectives: To determinate the threshold value of the immature platelet fraction IPF(%) indicator in predicting platelet recovery in thrombocytopenia dengue fever patients. Materials and methods: A cross-sectional descriptive study analyzed on 84 patients ≥ 16 years old diagnosed with dengue, with positive results of the Dengue virus NS1Ag rapid test or positive results  of the antibody dengue virus rapid test (IgG/IgM), a total peripheral blood cell analysis test with platelet count <150 G/L of blood. Platelet counts and IPF(%) values of all these patients were assessed daily. Results: Median peak IPF(%) among dengue patients was 14.0% with 88.1% of them achieving platelet recovery reaching the peak IPF (%). ROC curve analysis showed IFP% of 10.9% can be used to predict platelet recovery with a sensitivity of 79.73% and a specificity of 90.0%. Conclusions: IPF(%) is a novel and useful indicator in predicting platelet recovery in dengue patients. This will assist clinicians in monitoring and delaying unnecessary platelet transfusions in cases of severe thrombocytopenia dengue.
