CLINICAL CHARACTERISTICS AND OUTCOMES OF LAPAROSCOPIC CHOLECYSTECTOMY FOR ACUTE CHOLECYSTITIS IN PATIENTS WITH CHRONIC DISEASES

Abstract
Background: Cholecystitis one of the most common abdominal emergencies throughout the globe, including Vietnam. Chronic diseases impair the physiological function of the organs, the protective response, and the body's capacity to self-regulate, compared to healthy individuals. Current studies indicate that the prognosis of laparoscopic surgery in patients with acute cholecystitis and chronic disease is inferior to that in healthy subjects. Objectives: To describe the clinical characteristics and outcomes of laparoscopic cholecystectomy for acute cholecystitis in patients with chronic diseases in the Department of General Surgery, Can Tho Central General Hospital. Materials and methods: A cross-sectional descriptive study on 90 patients diagnosed with acute cholecystitis and chronic diseases undergoing laparoscopic cholecystectomy at Can Tho Central General Hospital from July 2022 to April 2023. Results: The average age of the patients was 65±13.9. Hypertension was the most prevalent chronic disease, accounted for 93.3%. With 58.9%, right hypochondrial discomfort was the predominant location. The ratio of fever, right hypochondrial tenderness, and localized pain were 81.1%, 82.2%, and 17.8%, respectively. Gallstones were responsible for most cases of acute cholecystitis (87.8%). The most frequently observed characteristics were enlarged gallbladder (90%) and thick wall (97.8%). The average hospital stay following surgery was 4.8 ± 1.4 days. Conclusions: Even in patients with comorbid chronic conditions, laparoscopic cholecystectomy is a safe and efficacious treatment for acute cholecystitis.
