CLINICAL, LABORATORY FEATURES AND COMPLICATIONS IN CHILDREN WITH DENGUE HEMORRHAGIC FEVER ASSOCIATED WITH LIVER IMPAIRMENT AT CAN THO CHILDREN’S HOSPITAL

Abstract

 Background: Liver impairment in Dengue hemorrhagic fever (DHF) causes coagulation disorders and many complications, making the condition longer and more severe. Objectives: Identify the clinical, laboratory features and complications in patients with DHF associated with liver impairment. Materials and methods: This was a cross-sectional study in which we collected data of all the patients in Can Tho Children’s Hospital in the period from  April 2018 to May  2020. Results: Among 230 pediatric patients, liver impairment were 35.2%. The most common clinical findings in DHF associated with liver impairment were fever (100%), restless/lethargy/drowsy (42%), abdominal pain (86.4%), liver enlargement >2cm (86.4%), vomiting (30.9%), petechiae (79%), mucosal hemorrhagic (24.7%), gastrointestinal bleeding (9.9%). The commonest abnormality laboratory results were increased AST/ALT level of at least twice higher than the upper limit of normal. The incidence of complications in Dengue patients associated with liver impairments were:  Shock (70.4%), hypoalbuminemia (75.3%), electrolyte disturbances (71.6%), bleeding (28.4%), coagulation disorders (75.3%) and disseminated intravascular coagulation (DIC) (51.9%). DHF  associated with liver impairment had more complications (p<0,05). Conclusion: The clinical, laboratory features and the occurrence of complications of Dengue in the group of liver impairment tended to be worse than the group without liver impairment.
