LABORATORY PREDICTORS OF SEVERE DENGUE AT CAN THO CHILDREN’S HOSPITAL

Abstract
Background: Prediction of severe dengue is essential for timely treatment to reduce mortality. The purpose of this study was to determine laboratory predictors of severe dengue. Materials and methods: A prospective cohort study of hospitalized dengue cases at Can Tho Children's Hospital from May 1, 2018 to May 1, 2020. Using a multivariate logistic regression model to determine laboratory indicators of significance to predict severe dengue and using ROC curve to evaluate the predicted values and calculate the sensitivity and specificity of each laboratory indicator. Results: A total of 230 dengue patients, 69 children with severe dengue and 161 non-severe dengue included in the analysis. Laboratory indicators had the predicted value of severe dengue in multivariate analysis included decreased serum albumin, prolonged APTT time, increased AST, increased ALT, thrombocytopenia and increased INR. The area under the ROC curve of these indicators were 0.96; 0.94; 0.94;0.89; 0.88 and 0.84, respectively. Conclusion: Our findings suggest that thrombocytopenia, hypoalbuminemia, prolonged APTT time, increased INR and increased ALT/AST are significant predictors of severe dengue in children.
