CURRENT STATUS AND RISK FACTORS OF ACUTE KIDNEY INJURY IN CIRRHOTIC PATIENTS AT CAN THO CENTRAL GENERAL HOSPITAL

Abstract

Background: Acute kidney injury is associated with a poor prognosis and is considered an important predictor of mortality in cirrhotic patients. Objectives: 1. Determining the rate of damage and stage of acute kidney injury in patients with decompensated cirrhosis. 2. Investigating risk factors for development of acute kidney injury in patients with decompensated cirrhosis at the Department of Gastroenterology of Can Tho Central General Hospital in 20192020. Materials and methods: 250 decompensated cirrhosis patients were admitted to the Department of Gastroenterology of Can Tho Central General Hospital, and prospective descriptive cross-sectional study. Results: Among 250 patients hospitalized for decompensated cirrhosis, there were 64 patients with acute kidney injury (about 25.6%). The risk factors associated with an increased incidence of acute kidney injury in patients with decompensated cirrhosis were history use of NSAID (OR=5.5, p=0.007), the use of diuretics (OR=12.2, p<0.0001), the use of ACE inhibitors (OR=6.5, p=0.003), infection (OR=2.1, p<0.02), and hepatic encephalopathy (OR=3.04, p=0.001) vomiting diarrhea (OR=1.7, p=0.5). Conclusions: The incidence of acute kidney damage in decompensated cirrhotic patients in our study was 25.6%. These are associated with increased rates of acute kidney damage in patients with decompensated cirrhosis: history of NSAID use, the use of diuretics, the use of ACE inhibitors, infection, hepatic encephalopathy. Acute renal injury is correlated with more severe or fatalities in patients with decompensated cirrhosis.
