CLINICAL, PARACLINICAL FEATURES AND OUTCOMES OF RADICAL SURGERY FOR STAGE I-IIIA PRIMARY OVARIAN CANCER AT CAN THO ONCOLOGY HOSPITAL
Main Article Content
Abstract
Background: Ovarian cancer is one of the most common gynecologic malignancies and is associated with high mortality. Radical surgery plays an important role in the treatment of ovarian cancer and in prolonging patient survival. Objectives: To describe the clinical and paraclinical characteristics and evaluate the outcomes of radical surgery for primary ovarian cancer at stages I– IIIA. Materials and Methods: A cross-sectional descriptive study combined with a non-controlled clinical intervention was conducted in 31 patients with primary ovarian cancer who underwent radical surgery at Can Tho Oncology Hospital from January 2025 to May 2026. Results: The mean age was 55.3 ± 12.4 years, with stage I disease accounting for the majority (54.8%). Lower abdominal pain was the most common presenting symptom (77.4%), and elevated CA-12.5 levels were observed in 83.9% of patients. Contrast-enhanced computed tomography (CT) of the abdomen and pelvis showed a mean tumor size of 14.3 ± 5.9 cm, with mixed solid-cystic tumors being the predominant morphology (67.7%). The concordance rate between intraoperative frozen-section examination and final postoperative histopathology was 87.1%. Epithelial ovarian tumors accounted for 83.9% of cases. The mean operative time was 110 ± 43 minutes. Operative time was longer in patients with stage II disease than in those with stage I disease and was not associated with tumor size. No intraoperative complications were recorded. Postoperative complications included abdominal surgical-site infection (3.2%) and vaginal cuff infection (3.2%); both responded to medical treatment. According to the Clavien–Dindo classification, surgical outcomes were rated as good in 93.5% of patients and fair in 6.5%, with no moderate or poor outcomes. Conclusion: Primary ovarian cancer at stages I–IIIA predominantly occurs in older women and commonly presents with lower abdominal pain, elevated CA-12.5 levels, and mixed tumor morphology on imaging. Radical surgery is safe and associated with favorable outcomes, a low complication rate, and no recorded intraoperative complications.
