CLINICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF KELOIDS AND HYPERTROPHIC SCARS WITH TRIAMCINOLONE INTRALESIONAL INJECTION AT CAN THO HOSPITAL OF DERMATO-VENEREOLOGY IN 2025

Abstract

Background: Keloids and hypertrophic scars are common pathological scars resulting from abnormal wound healing. They may cause deformity of the affected area, restrict function due to tissue contracture, and adversely affect patients’ psychological well-being and quality of life. Intralesional triamcinolone injection is a widely used treatment modality. The therapeutic response to intralesional triamcinolone is believed to be influenced by certain clinical characteristics. Objectives: Clinical characteristics of keloids, hypertrophic scars of patients treated with triamcinolone intralesional injection at Can Tho Hospital of Dermato-Venereology in 2025. Materials and methods: Crosssectional descriptive study was carried out on 93 patients with keloids, hypertrophic scars at Can Tho Hospital of Dermato-Venereology from January 2025 to October 2025. Results: The median age was 28 years (8-70); 61.3% were aged 15–35 years and 54.3% were female. Keloids accounted for 61.2% and hypertrophic scars for 38.8%. Most of the scars were spontaneous (48.4%) and had a scar area of <5 cm², accounting  for 61.3%. The vascular structures observed via dermoscopy in the keloid group were significantly higher than those in the hypertrophic scar group (93.0% compared to 61.1%). The mean Vancouver Scar Scale score significantly decreased from 6.48 ± 2.10 to 2.55 ± 1.81 after 24 weeks, and most of the differences between time points were statistically significant with p<0.05. Signs observed under dermoscopy tended to improve in parallel with the clinical response, especially the marked reduction in observable blood vessels, with the rate of vascular presence decreasing from 53.8% to 8.6%. Conclusion: Most of the scars were spontaneous, scar area <5 cm². The mean Vancouver Scar Scale score decreased after 24 weeks. Dermoscopy revealed that the scar features improved in parallel with the clinical response.
