ASSOCIATION BETWEEN NEWS2 RISK STRATIFICATION AND EARLY OUTCOMES (INTER-HOSPITAL TRANSFER AND MORTALITY WITHIN 24 HOURS OF ADMISSION) IN THE EMERGENCY DEPARTMENT AT CHAU DOC CITY MEDICAL CENTER
Main Article Content
Abstract
Background: The NEWS2 score is a standardized tool for early risk stratification based on routine physiological parameters. However, evidence regarding the association between NEWS2based risk stratification and early clinical outcomes among patients admitted to emergency departments at primary healthcare facilities remains limited. Objectives: To evaluate the association between NEWS2 risk categories and early outcomes, including inter-hospital transfer and mortality within 24 hours of admission in patients presenting to the Emergency Department. Materials and methods: An analytical cross-sectional study with longitudinal follow-up was conducted on 412 patients aged ≥18 years who were admitted to the Emergency Department of Chau Doc Medical Center between October 2025 and April 2026. The association between NEWS2 risk categories and early clinical outcomes was evaluated using logistic regression, with Firth's penalized likelihood correction applied when necessary. Results: The median NEWS2 score at admission was 4. Patients in the low-risk group accounted for 63.35%, the medium-risk group for 25%, and the high-risk group for 11.65%. Within 24 hours, the rates of inter-hospital transfer, mortality, and continued in-hospital treatment were 9.5%, 0.5%, and 90.0%, respectively. All deaths occurred in the high-risk group. Compared to the low-risk group, medium- and high-risk patients were 10.1 times (95% CI: 3.6–28.3; p <0.001) and 30 times (95% CI: 10.3–87.4; p <0.001) more likely to be transferred, respectively. For the composite adverse outcome, the odds were 9.4-fold (95% CI: 3.5–25.4; p <0.001) and 30.8-fold (95% CI: 11.1–85.5; p <0.001) higher in the medium- and high-risk groups, respectively. Conclusions: NEWS2 risk stratification is associated with early adverse outcomes within 24 hours. NEWS2 is a simple tool that can support risk stratification in district-level emergency settings.
