INVESTIGATING THE PREVALENCE AND ASSOCIATION OF HELICOBACTER PYLORI cagA AND iceA GENES WITH GASTRITIS AND PEPTIC ULCER DISEASE

Abstract

Background: The roles of cagA and iceA genes in the development of Helicobacter pylori (H. pylori)- induced gastritis and peptic ulcer disease remain unclear. Objectives: To determine the prevalence of H. pylori cagA and iceA genes and to investigate the association between these genes with gastritis and peptic ulcer disease. Materials and methods: A cross-sectional descriptive study was conducted on 69 patients with gastritis and peptic ulcer disease infected with H. pylori. The study was carried out at Can Tho Central General Hospital and Can Tho University of Medicine and Pharmacy Hospital from July 2025 to January 2026. The cagA and iceA genes were identified using PCR from gastric mucosa specimens. Results: The prevalence of cagA-positive and iceA-positive genes were 79,7% and 73,9% (37,7% iceA1 and 36,2% iceA2) respectively. Three genotypes of the iceA2 gene were identified: 24 aa, 59 aa, and 94 aa, with the 59 aa being the most common (72%). The presence of the iceA1 gene was significantly associated with peptic ulcers (Adjusted OR = 6,05; 95% CI: 1,40 - 26,18). Conclusions: The prevalence of H. pylori strains carrying cagA and iceA genes is high (79,7% and 73,9%, respectively). The iceA1 gene is associated with peptic ulcer disease.
