Abstract

Background: Decreased serum bicarbonate is a common condition in patients with endstage chronic kidney disease, which accelerates disease progression and increases the risk of other complications. Determining the prevalence of decreased bicarbonate concentration and its associated factors helps clinicians develop timely intervention strategies. Objectives: To determine the prevalence of decreased serum bicarbonate concentration and investigate associated factors in patients with stage 4 and 5 chronic kidney disease. Materials and methods: An analytical crosssectional study was conducted on 53 outpatients with stage 4 and 5 chronic kidney disease at Can Tho Central General Hospital from October 2025 to May 2026. Decreased serum bicarbonate was defined as an HCO3- concentration <22mmol/L. Results: The prevalence of patients with decreased serum bicarbonate was 54.7%. Through multivariable regression analysis, independent factors increasing the risk of decreased bicarbonate included: stage 5 chronic kidney disease (OR=3.167; p=0.042), urea (OR=1.076; p=0.029), creatinine (OR=1.003; p=0.012), and serum chloride (OR=1.189; p=0.048). Conversely, an increase in hemoglobin concentration was an independent factor reducing this risk (OR=0.636; p=0.009). Conclusions: The prevalence of decreased bicarbonate in patients with stage 4 and 5 chronic kidney disease is 54.7%. Stage 5 chronic kidney disease, elevated urea, creatinine, and serum chloride, along with decreased hemoglobin, are factors associated with decreased bicarbonate concentration.
