PROGNOSTIC VALUE OF THE SNAP-II SCORE FOR NEONATAL MORTALITY AT CAN THO CHILDREN’S HOSPITAL

Abstract
Background: Neonatal mortality remains a major challenge in neonatal intensive care units. The SNAP-II score is widely used to assess physiological severity and predict early neonatal mortality. Although several studies have evaluated its prognostic value in Vietnam, no study has been conducted in the Mekong Delta. Objectives: To evaluate the prognostic value of the SNAP-II score for predicting neonatal mortality at the Neonatal Department of Can Tho Children's Hospital. Materials and methods: A cross-sectional analytical study was conducted on 84 neonates admitted to the Neonatal Intensive Care Unit, Can Tho Children's Hospital, from January 2025 to February 2026. SNAP-II scores were recorded within the first 24 hours after admission. Prognostic performance was assessed using receiver operating characteristic (ROC) analysis and multivariable logistic regression. Results: The mortality rate was 20.2%. SNAP-II scores were significantly higher in non-survivors than in survivors (12 versus 5 points; p=0.001). The area under the ROC curve was 0.734 (95% CI: 0.591–0.877; p=0.003). The optimal cut-off value was ≥ 9.5, with a sensitivity of 58.8% and a specificity of 79.1%. After adjustment for sex and prematurity, a SNAP-II score ≥ 9.5 was independently associated with mortality (adjusted OR=5.27; 95% CI: 1.68–16.54; p=0.004). Conclusion: The SNAP-II score is an independent predictor of neonatal mortality and can support early risk stratification in neonatal intensive care units.
