Abstract

Background: Pneumonia is the leading cause of mortality in children under 5 years of age. However, current severity assessment scoring systems are primarily designed for adults. The Respiratory Index of Severity in Children (RISC) is a specialized tool, but data on its application in Vietnam remains limited. This study aims to evaluate the predictive value of the RISC score for disease severity in pediatric pneumonia patients aged 2 months to 5 years. Objectives: To evaluate the predictive value of the RISC scale in determining the severity of community-acquired pneumonia in children aged 2 months to 5 years at a tertiary pediatric hospital in the Mekong Delta region.  Materials and methods: A cross-sectional descriptive study with analysis was conducted on 120 children diagnosed with pneumonia and severe pneumonia, who were treated at Can Tho Children's Hospital from November 2024 to November 2025. Results: Male patients accounted for 55.8%, and the 12-60 month age group made up 56.7%. The proportion of severe pneumonia was 11%. A 1point increase in the RISC score was associated with a nearly 2-fold increased risk of progression to severe pneumonia (OR = 1.96; p = 0.003). The RISC score has predictive value for severe pneumonia with an area under the curve (AUC) of 0.746; at the cut-off value of 1 point, the sensitivity and specificity were 84.6% and 64.5%, respectively. Conclusion: The RISC score has good value in predicting the severity of pneumonia in children.
