CLINICAL OUTCOMES OF RESTORING MANDIBULAR MOLARS WITH MONOLITHIC GLASS-CERAMIC ENDOCROWNS

Abstract

Background: Restoration of endodontically treated mandibular molars using glass-ceramic endocrowns has gained increasing attention due to its tooth structure preservation and enhanced retention. However, follow-up data in Vietnam remain limited. Objectives: To evaluate the shortterm clinical outcomes of monolithic glass-ceramic Endocrown restorations for mandibular molars. Materials and methods: A single-arm clinical trial was conducted on 30 patients with 40 mandibular molars restored with monolithic lithium disilicate glass-ceramic Endocrowns from June 2025 to February 2026 at the Department of Dentistry, Phuc Nguyen 1 Dental Clinic, Buon Ma Thuot, Vietnam. Clinical evaluations using the United States Public Health Service (USPHS) criteria were performed at 3 and 6 months, assessing retention, structural integrity, marginal discoloration, marginal adaptation, color match, and overall treatment outcomes. Results: Immediately after cementation, 100% of restorations were rated Alpha for color match and marginal adaptation. At 3 months, the Alpha rates for retention and structural integrity were both 100%. At 6 months, the Alpha rate for retention decreased to 95.0%, while structural integrity remained at 100%. The Alpha rate for marginal discoloration was 100% at 3 months and 97.5% at 6 months, and the Alpha rate for marginal adaptation was 95.0% and 87.5%, respectively. The overall success rate was 100% at both follow-up periods. At 3 months, 95.0% of restorations were classified as Good and 5.0% as Fair, at 6 months, 87.5% were Good and 12.5% were Fair. No failures were recorded, and no statistically significant differences were observed between the two follow-up periods (p > 0.05). Conclusions: Restoration of mandibular molars using monolithic glass-ceramic Endocrowns demonstrated favorable initial clinical outcomes during the 6-month follow-up period.
