THE OUTCOMES OF MINIMALLY INVASIVE PLATE OSTEOSYNTHESIS IN THE TREATMENT OF MIDSHAFT CLAVICLE FRACTURE

Abstract

 Background: Clavicle fractures are among the most common fractures of the upper extremity. Open reduction and internal fixation have demonstrated earlier fracture union and a lower rate of nonunion compared with conservative treatment. However, conventional open surgery with a long incision is associated with certain complications. Minimally invasive locking plate osteosynthesis (MIPO) can mitigate these complication rates due to smaller incisions, less soft-tissue disruption, and better preservation of periosteal blood supply. Objectives: To evaluate the outcomes of minimally invasive locking plate osteosynthesis under fluoroscopic guidance in the treatment of midshaft clavicle fractures. Materials and methods: A prospective cross-sectional descriptive study was conducted on 15 patients with closed midshaft clavicle fractures. All patients underwent MIPO fixation using locking plates between February 2025 and December 2025. Results: The mean age was 39.7 ± 11.3 years. The average incision length was 2.5 ± 0.5 cm. All patients achieved solid bone union at 6 months postoperatively. The mean Constant-Murley score was 86.5 ± 5.8, indicating good functional recovery. Conclusion: Minimally invasive locking plate osteosynthesis under fluoroscopic guidance is a safe and effective technique. The MIPO technique preserves the periosteal blood supply is critical for maximizing osteogenesis and ensuring optimal cosmetic outcomes.
