MICROBIAL ETIOLOGY OF PNEUMONIA IN CHILDREN WITH ACUTE MALNUTRITION AGED 2 MONTHS TO 5 YEARS AT CAN THO CHILDREN’S HOSPITAL, 2024–2026

Abstract
Background: Pneumonia is a leading cause of death among children under five years of age. Malnutrition impairs immune function, leading to more severe disease progression and an increased risk of treatment failure. In Can Tho, studies on the microbial etiology of pneumonia in children with acute malnutrition identified using molecular diagnostic techniques remain limited. Objectives: 1. To describe the clinical and paraclinical characteristics of pneumonia in children aged 2 months to 5 years with acute malnutrition. 2. To determine the prevalence of microbial pathogens causing pneumonia in the study population. Materials and methods: This cross-sectional descriptive study included 52 children aged 2 months to 5 years diagnosed with pneumonia and acute malnutrition at Can Tho Children’s Hospital from 2024 to 2026. Clinical and paraclinical characteristics were collected. Microbial pathogens were identified using Real-time PCR and bacterial culture techniques. Results: Most children were in the 1–5-year age group (94.2%). Tachypnea was the most common symptom (84.6%), and mixed crackles were the predominant lung auscultation finding. Regarding laboratory findings, 55.8% of children had CRP levels ≥10 mg/L, while most had white blood cell counts <15,000 cells/mm³ (82.7%). The most frequently detected bacterial pathogen was non-typeable Haemophilus influenzae (NTHi) (71.2%). Cytomegalovirus (CMV) was the most frequently detected virus (23.1%). Conclusions: Non-typeable Haemophilus influenzae was the most frequently detected bacterial pathogen among children with pneumonia and acute malnutrition. Bacterial–viral coinfection was common in this population.
