Abstract

Background: Restoration of endodontically treated molars remains a clinical challenge due to loss of tooth structure and risk of fracture. Semi-direct composite onlay/overlay restoration is a conservative and cost-effective approach to reinforce cuspal coverage while preserving remaining tooth structure. Objectives: To describe the clinical characteristics and evaluate the restorative outcomes of endodontically treated molars restored with semi-direct composite onlays/overlays. Materials and methods: This prospective, single-arm clinical interventional study with 6-month follow-up period included in 60 endodontically treated molars restored using the semi-direct composite onlay/overlay. Clinical procedures were standardized. Results: Performance across all criteria remained promising and stable, achieving an overall success rate of 98.33%, only 1 failure out of 60 cases after 6 months. Conclusions: Semi-direct composite onlay/overlay demonstrate favorable short-term clinical outcomes, represents an effective conservative treatment and costefficient option for restoring  function and esthetics in endodontically treated molars.
