CLINICAL AND PARACLINICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF ADULT PATIENTS WITH DENGUE HEMORRHAGIC FEVER AT CAN THO GENERAL HOSPITAL

Abstract

Background: Dengue hemorrhagic fever presents with a wide range of clinical manifestations. Therefore, understanding the actual clinical features of dengue hemorrhagic fever in adult patients is essential for early diagnosis and timely treatment, thereby reducing patient mortality. Objectives: 1) To describe certain clinical and paraclinical characteristics of adult patients with dengue hemorrhagic fever at Can Tho City General Hospital; 2) To evaluate the treatment outcomes of adult patients with dengue hemorrhagic fever. Materials and methods: A cross-sectional descriptive study was conducted on 125 patients diagnosed with dengue hemorrhagic fever and hospitalized in the Department of Infectious Diseases at Can Thơ City General Hospital from May 2024 to May 2025. Results: 41.6% had dengue fever (DF), 50.4% had dengue fever with warning signs, and 8% had severe dengue. Clinically, fever was present in 100% of patients, followed by headache (91.2%), arthralgia (53.6%), hepatic pain (51.2%), persistent vomiting (48%), and shock (6.4%). Laboratory findings: Hct >40% peaked on day 4 (42,16%). Platelets decreased on days 2–5 and increased by day 10 but remained <100.000/mm³ (93.000/mm³). WBC <4.000/mm³ from day 2 and increased from day 5 (4.910/mm³). Mild acute liver injury occurred (AST, ALT: 120 - <400 U/L)  in 24.8%, moderate (AST, ALT: 400 - <1000 U/L) in 8%, and severe (AST, ALT: >1000 U/L) in 0.8%. Pleural effusion and ascites were noted in 31.2% and 32.8% of patients, respectively. The NS1 antigen was positive in 60.8%, and IgM/IgG antibodies were positive in 39.2% of patients. The mean duration of hospitalization was 5.15 ± 1.46 days, and all patients were successfully treated. Conclusion: Clinical manifestations of dengue hemorrhagic fever in adults were diverse, ranging from dengue with warning signs to severe dengue. The treatment outcomes were favorable, with a high recovery rate among hospitalized patients (100%).
