THE STUDY ON CHARACTERISTICS OF ANTIBIOTICS USE AND ASSESSMENT OF THE APPROPRIATE ANTIBIOTICS USE IN PATIENTS WITH PNEUMONIA AT A HOSPITAL IN CAN THO CITY 2018-2020

Abstract

 Background: In the current situation of antibiotic resistance, determining causative pathogens and evaluating prescription antibiotics therapy help physicians reach appropriate antibiotic regimens and treatment efficiency and mitigate the percentage of antimicrobial resistance. Objectives: To identify the characteristics of antibiotic use and assess the appropriateness of antibiotics therapy in inpatients with community-acquired pneumonia and hospital-acquired pneumonia following the Ministry of Health guidelines. Materials and methods A cross-sectional study was conducted on 155 patients diagnosed with pneumonia who were collected sputum specimens for microbiological tests at a hospital in Can Tho city from 04/2018 to 04/2020. Antibiotic therapy before and after microbiological results was appropriate when all 4 appropriate criteria in terms of antibiotic selection, dose, route of administration, and the number of times. Antibiotic selection was the first criterium to be evaluated, and other criteria were evaluated after appropriate antibiotic selection. Data were collected from medical records and were analyzed by SPSS 18.0 software. Results: Combination of β-lactamase with quinolones reached the highest rate at 77/155 (49.7%), following cephalosporin with 46/155 (29.7%) and β-lactamase/ βlactamase inhibitor combination with 18/155 (11.6%). Antibiotic therapy before and after microbiological results appropriate to guidelines accounted 49% and 65.9% relatively. While the appropriate number of antibiotics in initial therapy was relatively low and reached 44.5%, that of the antibiotic therapy after microbiological results rose significantly, reaching 65.9%. Conclusion: The percentage of antimicrobial regimens following the guidelines of the Ministry of Health was relatively smaller than other criteria. Microbiological tests have contributed to improving the rate of appropriate antibiotic use.
