CLINICAL CHARACTERISTICS, RADIOLOGICAL FEATURES AND TREATMENT OUTCOMES WITH POSTERIOR LUMBAR INTERBODY FUSION (PLIF) AT CAN THO UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL

Abstract

Background: Spondylolisthesis is anteriorly slippage of one vertebral body with its pedicle, transverse process and superior articular facet. Surgery is indicated when there is instability or failure of conservative treatment. Objectives: To describe clinical characteristics, radiological features of lumbar spondylolisthesis and evaluating treatment outcomes of posterior lumbar interbody fusion at Can Tho University of Medicine and Pharmacy hospital. Materials and methods: Cross sectional study of 34 patients diagnosed with lumbar spondylolisthesis underwent PLIF at Can Tho University of Medicine and Pharmacy Hospital from January 2018 to March 2020. Results: Mean age of the study was 58.48 years (22-78), women more/ men ratio was 1.43:1. Frequent symptoms were back pain (100%), neurogenic claudication (100%), nerve root compression signs (70.6%), tenderness (64.7%). Spondylolisthesis commonly occured at L4/L5 level (41.2%), L5/S1 level (23.3%), L3/L4 level (8.82%), 8 cases (23.3%) with double-level spondylolisthesis. Primary slippage degree was grade I (85.3%). Mean surgical time was 159.4 minutes (80-255), mean blood loss was 460 ml (250-1000), there were 16 (47.1%) cases needed blood infusion. Post-operatively, all patients experienced symptomatic improvement (both back VAS and lower extremity VAS). One got root injury and fully recovered, one superficial wound infection, no instrument breakage. At a follow-up after 12 months (21 cases), excellent rate of bone fusion was 67.7%, good rate was 33.3%, no osteolysis. Final results: excellent 41.2%, good 52.9%, moderate 5.98%, no poor result. Conclusions: Posterior Lumbar Interbody Fusion was a safe procedure with good outcomes for the treatment of lumbar spondylolisthesis.
