THE SITUATION, CLINICAL, SUBCLINICAL CHARACTERISTICS AND TREATMENT RESULTS OF POSTPARTUM HEMORRHAGE FOLLOWING VAGINAL DELIVERY AT VINH LONG GENERAL HOSPITAL

Abstract

Background: Postpartum hemorrhage (PPH) is one of the leading causes of morbidity and mortality in Vietnam and all over the world. Despite the remarkable improvement of medical staff qualifications and medical facilities, PPH is still the most dangerous obstetric complication for obstetricians. Objectives: Describe the incidence and causes of PPH; To survey the clinical, subclinical characteristics and evaluate the results of postpartum hemorrhage treatment. Materials and methods: 86 pregnant women with vaginal deliveries at Vinh Long General Hospital or transferred from other hospitals to Vinh Long General Hospital from February 2019 to June 2020. Results: The incidence of PPH was 7.1%. The percentage of PPH due to uterine atony was 82.6%, vaginal injury was 15.1% and retained placenta was 2.3%. Major occurred in women aged 20-35 years, accounting for 73.3%. Mean pulse rate was 85.7 per minute, mean systolic blood pressure was 107.1mmHg, mean diastolic blood pressure was 63.8mmHg. The average gestational age was 38.1 ± 4.25, from 36 weeks to 41 weeks, gestational age from 38-40 weeks accounted for 73.3%. 93% of PPH was moderate blood loss, 96.5% after vaginal delivery and 98.8% of PPH was detected within 2 hours after. There was a change in blood count as erythrocytes, hemoglobin and hematocrit in PPH. Results of PPH treatment: The percentage of successful treatment was 96.5% and only 3.5% had to do hysterectomy. Conclusion: Most of PPH was detected soon and treated successfully.
