Abstract

 Background: Diabetes mellitus (DM) is the third leading cause of mortality in Vietnam. Achieving optimal glycemic control is essential in diabetes management, and medication adherence plays a crucial role in reaching therapeutic goals and preventing long-term complications. Objectives: 1. To determine medication use characteristics; 2.  To determine the rate of medication non-adherence and identify associated factors among outpatients with type 2 diabetes at Chau Doc General Hospital during 2025–2026. Materials and methods:  A cross-sectional descriptive study was conducted on 188 insured outpatients with type 2 diabetes from July 2025 to January 2026. Data were collected through medical record review and patient interviews, and analyzed using SPSS version 20.0. Results: More than half of the participants were older than 60 years (55.3%) and female (59.0%), resided in rural areas (66.5%), and completed general education (64.9%). A disease duration of ≥10 years was observed in 45.2% of patients. Regarding medication use, 76.6% of patients used ≥5 medications, and 55.9% of prescriptions did not include insulin. The overall medication nonadherence rate was 31.9%. Statistically significant associations were identified between nonadherence and the following factors (p < 0.05): Educational level, occupation, disease duration, treatment duration, insulin use. Conclusions: Medication non-adherence among T2DM outpatients remains high and is multifactorially influenced by both sociodemographic and clinical attributes The findings highlight the need to strengthen counseling, follow-up, and personalized health education to support patients in improving adherence and achieving better long-term treatment outcomes.
